Your tasks:
- Individual psychosocial counseling and support for senior citizens in personal problem or emergency situations
- Management of personal upheaval situations and changes including case management with needs assessment, target definition and review of the effectiveness of the agreed measures
- Mediation of support options for person-specific needs or problem situations, e.g. addiction or other dependencies, need for care, domestic care, loneliness
- Supporting senior citizens with the organization of everyday requirements, applications, correspondence, contacts with authorities, etc.
- Carrying out and updating the neighborhood-related social space analysis in coordination with superiors
- Establishment, organization and professional support of low-threshold meeting places and meeting opportunities in the neighbourhood
Your profile:
- Completed studies as a graduate social pedagogue (FH) or Bachelor of Social Work, preferably with state recognition
- Knowledge of the legal framework of SGB XII is desirable
- Experience in support and counseling work as well as in project and network work is an advantage
- Communication skills, empathy and decisiveness
